Answer and Explanation:

The Correct Answer is C.

When issues are found while project work is being performed, change requests are issued. These may modify project policies or procedures, project scope, project cost or budget, project schedule, or project quality. Other change requests cover needed preventive or corrective actions to minimize negative impact later in the project. Your first step should be to get the approval for the defect repair from the Change Control Board (CCB). In order to do this you need to issue a change request first. By the way: Did the term "civil work" in the question confuse you because you didn't exactly know what it meant? It wasn't necessary to understand what the exact nature of the project is or what the work performed was in order to answer this question correctly. All you needed to understand was that deliverables had defects that needed to be repaired in a controlled manner. Remember that the PMP Exam tests your understanding of project management concepts and not whether you know how to manage an IT project, construction project or "civil works" project.
